Alternative Services

Alternative Services (AS) is part of the A/E license that has an on-site capacity of 75 (this includes those served in Enhanced Services) and unlimited off-site capacity. We serve approximately 40 - 45 clients that range in age from 21 to 70 who have medical and/or self-care needs (i.e. diabetes, epilepsy, mobility issues, or visual and/or hearing impairments) or are not engaged by vocational activities. In this area we employ twelve staff, including two fulltime Certified Occupational Therapy Assistants, maintaining a 1:4 staff to client ratio. The focus of the AS area is:  direct and indirect therapies, social/leisure activities, community integration, self-advocacy, and production. Clients enjoy a weekly music session led by a music therapist and additional therapeutic activities that include: art, cooking, pets, and sensory. AS also enjoys primary access to the greenhouse and courtyard to support a horticulture therapy program and large group activities. This service is funded through CADI, CCSA, DD Waiver or Medicaid depending on the client's resources.

Enhanced Services

Enhanced Services (ES) is part of the A/E license which has an on-site capacity of 75 (includes those served onsite in Alternative Services) and unlimited off-site capacity.  We serve approximately 50 clients (30 on-site and 20 off-site) that range in age from 21 to 60 that have a history of intense maladaptive behaviors (e.g. elopement, physical aggression, property destruction, self-injurious behavior, etc.) or significant medical concerns. In this area we employ seventeen staff maintaining an enhanced staff to client ratio (i.e. 1:1, 1:2, 1:3, or 1:4); with smaller, quieter, and structured settings; and more individualized strategies to perform production tasks and participate in social/leisure activities. Most individuals work 10-20 hours per week and are paid a piece rate for work completed. With a philosophy of being prepared for daily challenges and achieving the best outcomes, the focus of the ES area is:  work production,off-site work crews, social/leisure activities (e.g. arts & Crafts, community activities, social skills games, etc.), positive behavior support planning, sensory integration, communication, therapies, and self-advocacy. The use of Rule 40 programs is almost nonexistent and we only average 6 – 10 emergency uses of a controlled procedure a year. This service is funded through CADI, CCSA, DD Waiver or Medicaid depending on the client's resources.

Individual Services

Individual Services (IS) is part of the U license and has unlimited capacity. We are serving more than 30 clients that range in age from 21 to 50 who require the least amount of support and are capable of independently following basic instructions. In this area we employ two fulltime staff that provide regular, intermittent, support to ensure that clients are meeting the employer's expectations. Clients are making at least minimum wage and paid directly by the employer, making it a great choice for clients funded through MA-EPD; and we use vendor agreements to provide additional support to clients when our staff are not present. These individuals work a minimum of 20 hours and up to 40 hours per week. The focus of the IS area is: job development, job placement, job performance, transportation, community integration, expanding social networks, and participating is self advocacy activities. This service is funded through CADI, CCSA, DD Waiver MA-EPD, or Medicaid depending on the client's resources.

Offsite Services

Offsite Services (OS) is part of the U license and has unlimited capacity. We are serving more than 100 on 22 work crews that range in age from 21 to 60 who require consistent support and supervision to complete their assignments. In this area we employ more than 25 staff that transport a crew of 3 – 8 clients to their work site and supervise their activities. These individuals work a minimum of 10 to 20 hours per week as part of a team to meet the needs of the bsuiness. The focus of the OS area is:  meeting employer expectations; participating in social/leisure activities (e.g. indoor/outdoor physical activities, tours, volunteerism, etc.); and participating in self-advocacy activities. This service is funded through CADI, CCSA, DD Waiver, or Medicaid depending on the client's resources.

Utility Services

Utility Services (US) is part of the U license which as an onsite capacity of 110 and unlimited offsite capacity. We serve approximately 90 clients that range in age from 21 to 80 who require less structured work and/or social environments, less staff supervision, and are capable of completing work tasks after being trained. We use the definition of "utility" found in Webster's Ninth New Collegiate Dictionary that states "capable of serving as a substitute in various roles" to describe the area's strength. In this area we employ more than 15 staff that support clients in completing light industrial jobs (e.g. assembly, collating, packaging, shredding, etc.) that are out-sourced from businesses. Most of these individuals work 15-20 hours per week and are paid a piece rate for work completed. The focus of the US area is: job assessment, training, and support; work production; and participation in self-advocacy activities. This service is funded through CADI, CCSA, DD Waiver, or Medicaid depending on the client's resources.

Adult Day Services

Adult Day Services (ADS) which has an onsite capacity of 18 and unlimited offsite capacity. The ADS program can serve up to 18 clients that must be seniors or individuals under the age of 65 with any of the following diagnoses:  dementia, physical disabilities, traumatic brain injuries, and developmental disabilities. A full day is considered to be five (5) hours, not including transportation time, and services are designed to meet the needs of clients only interested in non-vocational activities. Persons will be served who: cannot attend a senior citizen's center because of physical, mental, or emotional circumstances; may have challenges in leaving their home; need assistance with grocery shopping and meal preparation; require medication management; or have limited resources in terms of family/friends that can assist them. The target population is further defined as adults, who may be in need of protection and are vulnerable as defined in MN Statute CSSA section 626.557; who are not capable of fulltime independent living, and are experiencing difficulty providing for their own needs. The focus of ADS is: on- and offsite social/leisure activities (e.g. arts & crafts, horticulture, pet therapy, music therapy, cooking, social skills games, dining etiquette, shopping, tours); therapies (e.g. speech, OT); self-advocacy activities; health and fitness monitoring, current events, etc. This service is funded through CADI, CCSA, DD Waiver, Elderly Waiver, Medicaid, or TBI Waiver depending on the client's resources.

Supported Employment Services

Supported Employment Services (SES) has its own license and an unlimited offsite capacity. We are serving more than 20 clients that range in age from 21 to 50 who primarily rely on natural supports, with occasional follow-up by our staff, to be employed by a business. Clients are working a minimum of 20 hours and up to 40 hours per week, making at least minimum wage, and are paid directly by the employer. In this area we employ one fulltime staff that provides hourly services to clients in the following areas: job development, case management, initial job training and follow-up, and transportation coordination. The only service funded is employment and vendor contracts are not permitted. This service is funded through CADI, CCSA, DD Waiver, or Medicaid depending on the client's resources.

Ticket to Work

We are an "Employer Network" authorized by the Social Security Administration with an unlimited capacity to serve adults with developmental disabilities through the Ticket-To-Work (TTW) program. We are serving less than 5 people through the TTW program and a Service coordinator (QMRP) is responsible for overseeing the coordination of their services. The focus of TTW is for clients to have consistent employment so that persons no longer need SSI and SSDI benefits. Job development, case management, initial job training and follow-up, and transportation coordination is offered through this service. Payment is based on the type of ticket and personal milestones met.
